Here are three options according to the site owner. We recommend using the online removal form.
1. Option One: NEW: At 15:33 UTC on 7 April 2015, we began uploading individual detail pages containing a web form at the bottom of the page, where the person named on that page can submit a removal request on line. These seem to be working on our Colorado and Connecticut sites, so we are testing them on our Florida site (the desktop version, not the mobile-friendly version.)
Option Two: Download the .rtf or .txt file, get it notarized so we know it is really you, and mail it to Tom Alciere, 67 Technology Way, Nashua NH 03060 U.S.A.
Option Three: You may instead mail in a photocopy of the voter information card the elections people provided to you when you registered. Either way, you must spell the webmaster’s name (Tom Alciere) correctly.
